A housing association are currently looking for a Young Persons Support Worker to join their team on a temporary basis. As the Young Persons Support Worker you will provide intensive housing support for younger customers to increase levels of tenancies sustained and prevent homelessness.

You key duties as the Young Persons Support Worker are to:
* Manage your own caseload and prioritise referrals to ensure those customers most in need are offered services.
* Create and regularly review person centred support plans to monitor the progress of each customer and effectiveness of the intervention.
* Support individuals to achieve and maintain their tenancy, be actively involved in the development and delivery of their services and to achieve greater confidence and independence.
* Ensure all safeguarding policies and procedures are implemented at all times and that all safeguarding issues are managed effectively; by sharing good practice to minimise risk.
